Scheduling Your Exam

(Please do not book the appointment online).

Exam Fees:

• Initial visit: $225.

  • The $25 “scheduling deposit” will be applied to the initial visit.

• Pick up I-693 paperwork / Subsequent visit: $100.

  • Based on initial visit and/or outstanding requirements for I-693, you may be required to come back for a second visit

  • In most cases we should be able to give you the completed form during the second visit.

  • If your paperwork is incomplete during the second visit, you will be required to return for subsequent visits incurring $100 fees for each visit.

  • If you satisfy all your requirements and bloodwork is normal, the Civil Surgeon will complete the I-693 and you can pick up the sealed envelope (which still incurs a $100 fee per I-693)

Other Fees (If Applicable):

• Lab Requirements & Cost - $80 to $260

  • You can call your insurance company and ask them if the below tests will be covered and choose to use your insurance. ICD 10 codes used include Z11.7, Z11.3 and Z23. If you insurance company is asking for CPT codes, then you have to contact the lab (Labcorp or Quest).

  • If you choose to pay out of pocket for labs, please note that the payment is non-refundable if you do not go to the our partner lab (Labcorp).

  • Quantiferon TB (required).

  • RPR for Syphilis (based on age and clinical evaluation).

  • Neisseria gonorrhea Urine test (based on age and clinical evaluation).

  • MMR Titer (not required if you have proof of 2 doses of MMR vaccine, or lab result showing titer values, or for 65 years & older).

  • Varicella Titer (not required if you have proof of 2 doses of Varicella vaccine or lab result showing titer values).

  • Hepatitis B Titer (not required if you have proof of 3 doses of Hepatitis B vaccines, or lab results showing titer values, or over 59 years of age)

• Vaccine Requirements & Cost - $70 to $700

  • If you have insurance, you can get the vaccines at your PCP’s office or at a pharmacy.

  • The following vaccines are required for Adults (18 and above):

    • COVID-19 Vaccine (latest booster)

    • Tdap or Td

    • Influenza Vaccine (during influenza season generally September to March)

    • Varicella Vaccine or varicella titer results

    • MMR Vaccine or MMR titer results (Not required for 65 years and older)

    • Pneumonia Vaccine (Required for 65 years and older)

    • Hepatitis B Vaccine or Hepatitis B titer results (Not Required for Age 60 and older)

• Interpretation services - $50

  • If you don’t speak English and we use our “ interpretation service”, then there is an additional cost.

  • You are welcome to bring someone you trust to help interpret for you to save on this cost.

• Payment method: We accept cash, credit and debit card. We do not accept personal checks.

Please Note: Fees are non-refundable. You are paying for the Civil Surgeon’s time to perform the exam and complete paperwork.

What to bring during your exam:

• A valid passport or Federal government-issued photo ID.

  • Please note that if your Driver’s License says “Not for Federal Identification” we cannot use that as identification document for I-693

• Form I-693.

  • (Please complete your portion of the form).

• Proof of vaccines - the following information is required if vaccines were administered in USA:

  • Name of Vaccine

  • Date of Administration

  • Manufacturer

  • NDC Code

  • Lot Number

  • Expiration Date

  • If vaccines were given outside USA then either the original, or official documentation is required as proof.

  • If vaccine records are not in English, you will be required to bring official translation along with the original non-english documents.

  • You can also get proof of vaccine from CT State registry by going to the following website - https://ctwiz.dph.ct.gov/ctwiz_public/Application/PublicPortal

• COVID-19 Vaccine card (or official proof if from another country)

• List of any medications that you take regularly.

• If you have been treated for tuberculosis (TB), syphilis, or gonorrhea - a written certificate describing medications taken, start and stop dates of medications and lab/imaging results is required.

• If you have been treated for mental illness, alcohol or drug abuse - a written certificate indicating diagnosis, treatment, and prognosis is required.

Information About the Exam:

• You will be given an appointment date and time with the doctor.

  • Please arrive 15 min before your appointment time. The office staff will check you in, and you will be asked to complete screening and medical history forms.

• The doctor will review the your medical history, vaccination records and perform a standard physical exam

• Once we receive the results of your tests, we will schedule a follow up to review the test results, give additional vaccines if indicated and to complete, date and sign the form (additional cost noted above).

Following the above guidelines will reduce your total out of pocket costs.

Submitting the Results of Immigration Exam

After your exam is concluded, you will receive the completed Form I-693 with supporting documentation in a sealed envelope. Do not open or break the seal on this envelope. The sealed envelope must be submitted with your application to USCIS for processing.

A copy of the completed Form I-693 and lab results will also be given to you for your records.

Other Frequently Asked Questions:

• Why do I have to pay $100 to pick up I-693 or for a second visit?

  • The Civil Surgeon and office staff work “in the background” after your first visit to complete your I-693 and complete any additional documentation that is required (e.g. results of your bloodwork). The $100 charge is for this additional work.

    • If the Civil Surgeon is able to complete your I-693 then you can pick up I-693 at your convenience (we do not have to find a time slot in the Civil Surgeon’s schedule to fit you in).

  • If needed, we reserve a 15 minute time slot for the doctor to meet with you. During this appointment he will review additional paperwork including pending vaccine records. He will explain your test results and complete your I-693.

• Why do I have to pay a $25 deposit fee to schedule appointment?

  • We have a number of people who schedule an immigration exam and then do not show up.

  • The $25 deposit fee is to ensure that people keep their appointments.

  • The $25 will be applied towards the first visit.

  • For families scheduling immigration exam, a single $25 deposit applies to entire family.

• Are there any “No-Show” fees?

  • Yes. If you do not come for your appointment there is $50 “No-Show Fee”.

  • We also charge a $25 rescheduling fee if you reschedule your appointment within 1 business day or 24 hours (whichever is earlier)

    • If you have appointment on Monday, you need to cancel on Friday

    • If you have an appointment at 8:30 am Tuesday, you need to cancel when office opens on Monday morning (i.e. 24 hours before)

• Will I be charged “interpretation fees” if I don’t speak English?

  • We reserve the right to charge $50 interpretation fees if you do not speak English, or the person accompanying cannot speak English fluently.

  • If the doctor is unable to understand the person interpreting for you, he may decide to use the interpretor. This will be decided by the doctor during your appointment.
