Immigration Exams - Please don’t book online
The Civil Surgeon’s appointment (Dr. Sudeep Bansal) does not have online appointments.
Amy Close, APRN, and Robin Bansal, APRN, cannot do immigration exams. If you schedule with them, your appointment will be canceled and rescheduled.
Please call the office at (860) 430-5599 to schedule your immigration appointment.
The immigration exam is not covered by insurance.

Primary Care Appointments
Before booking online appointment, please review the instructions below to avoid last minute cancellations or change in your appointment.
If your insurance has changed, please follow instructions for “New Patient Appointments” below.
Please use your Full Legal Name to book appointments. This avoids creating “duplicate records” and may lead to errors in your care.
New Patient Appointments:
Please review the list of insurances that we accept on the Home Page before scheduling your appointment.
Please make sure we are in-network (even if we accept your insurance as your plan may still restrict the doctors you can see)
Please upload your insurance card prior to visit.
Please change your PCP to either Amy Close, APRN or Robin Bansal, APRN with your insurance company prior to scheduling.
If you have a different PCP listed with insurance company, your insurance may mandate a higher copay, deductible, or you be responsible for payment of entire visit.
Please Note that we do not prescribe these medications:
Narcotics/controlled substances pain medications
Benzodiazepines (Xanax, Ativan, clonazepam etc)
ADHD Medications (Adderall, Vyvanse etc)
We can work with you to taper these medications or refer you to appropriate specialist if needed.
Annual Physical
Annual Physicals are once every 365 days.
Before you book, please make sure that 1 year (365 days) have (or would have) elapsed since your last Physical.
Please do not book an Annual Physical, if you have medical issues that you would like to discuss.
Book a “Sick Visit” and describe your medical issue in the comments section
Please review “Annual Physical vs Medical Visit” and “High Deductible Health Plans.”
All insurance plans require us to charge a copay or deductible for a combined Annual Physical and Medical Visit.
New Pre-Op Exams
In addition to following instructions for “New Patient Appointments,” please follow the instructions below:
Please book as “Pre-Op” Visit, not “New Patient.”
Please make sure your surgery is within 30 days of your Pre-Operative exam.
If you have access to the Patient Portal, please schedule appointments from the Portal. This ensures that a “duplicate patient” account is not created.
